The Assess and Restore Collaborative (ARC) brings together partners who provide rehabilitative care to frail older adults throughout northeastern Ontario.

The ultimate intent of this project is the development and implementation of a standardized process to enable direct admission of frail older adults to facility-based rehabilitative care. This process is based on the pillars of screening, comprehensive assessment, and timely admission to rehabilitative care from the community.

Project Plan

Phase I of this project is focused on building the capacity to implement the priority process in the Sudbury and Manitoulin, and Algoma districts. Phase II will focus on measuring the outcomes in these communities, while expanding to other districts across the North East Local Health Integration Network (NE LHIN).

Education and Capacity Building

A fundamental component of this project is to advance expertise and build capacity for geriatric care in northeastern Ontario. A curriculum for healthcare providers will be compiled and shared to help build the capacity necessary for the successful implementation of this process.
